Specific details of the requirement
- Initiate the Models of Care Self Determination and Planning Process
- Convene a panel to facilitate the cultural authority in decision making regarding placement and care support.
- Facilitate:
o the panel’s development of cultural assessments to inform placement decisions following family meetings and family exploration
o designing of a care and support option that best meets the need of the child or young person
-Document the process of the panel and model including challenges, responses to challenges, reflections, to allow for replication elsewhere.
-Support the development of measures, outcomes and monitoring process.
